Former New York City Mayor Rudy Giuliani, once known as “America’s Mayor” and now a controversial figure in Donald Trump’s political orbit, was seriously injured Saturday night in a violent car crash on Interstate 93 in Manchester, New Hampshire.

According to New Hampshire State Police, Giuliani’s vehicle was “struck from behind at high speed” around 10 p.m. while he was riding in the passenger seat of a rental car. Both vehicles were “heavily damaged” and veered into the median.

Michael Ragusa, Giuliani’s longtime head of security, confirmed in a statement that the 80-year-old was rushed to a nearby trauma center, where doctors diagnosed him with a fractured thoracic vertebrae, deep lacerations, severe bruising, and injuries to his left arm and lower leg.

“This was a horrific crash,” Ragusa told the New York Post. “But the mayor is in great spirits. He’s a beast. He survived 9/11, and he’ll survive this.”

Just moments before the crash, Giuliani had stopped to help a woman on the highway who he believed was fleeing a domestic violence incident.

“Mayor Giuliani immediately rendered assistance and contacted 911,” Ragusa explained. “He stayed with the victim until police arrived to ensure her safety.”

State Police later confirmed that officers investigating the initial domestic violence situation were just across the highway when the collision occurred and witnessed the entire impact. First responders “crossed the interstate and rendered immediate aid” to Giuliani and two other injured individuals.

The crash comes at a pivotal moment for Giuliani, who has been mired in years of legal battles stemming from his role in Trump’s failed effort to overturn the 2020 election.

Once a celebrated hero of New York’s post-9/11 recovery, Giuliani’s reputation has plummeted. He has faced crippling financial penalties, including a $148 million defamation judgment won by two Georgia election workers he falsely accused of ballot tampering. He has also been permanently disbarred from practicing law in Washington, D.C., and faces ongoing lawsuits tied to election misinformation.

“He’s been through hell,” said one longtime associate who asked to remain anonymous. “But Rudy’s always been a fighter. That’s who he is.”

Within hours of the crash, wild conspiracy theories began spreading online, fueled by Giuliani’s ongoing entanglements in Trump’s legal and political battles. Ragusa moved quickly to shut them down.

“This was not a targeted attack,” he posted on X, formerly Twitter. “Mayor Giuliani was in a rental car. No one knew it was him. We’re asking people to respect his privacy and stop spreading unfounded rumors.”

Notably, as of Sunday morning, President Donald Trump has not publicly commented on his former attorney’s condition, despite their once-close alliance. In recent months, Giuliani has reportedly been on the outs with Trump’s inner circle as the former president faces multiple criminal investigations.

Political analysts say the incident could reignite scrutiny of Trump’s reliance on controversial allies like Giuliani, particularly as the administration grapples with declining approval ratings and ongoing legal crises.

“This is another reminder of just how chaotic Trump’s orbit has become,” said political strategist Maria Cardona. “Giuliani went from ‘America’s Mayor’ to a symbol of election denialism, and now he’s literally fighting for his life.”

Giuliani remains hospitalized as of Sunday evening and is undergoing “further tests and stabilization,” according to his partner and business associate Maria Ryan.

“He’s in pain, but he’s tough,” Ryan told the Post. “We’re taking things one step at a time.”

The New Hampshire State Police investigation into the crash is ongoing. Authorities have not yet released the names of the other drivers involved or indicated whether charges will be filed.
