Jill Biden is speaking with unusual candor about the frightening reality facing her family as former President Joe Biden continues his battle with an aggressive form of prostate cancer.

The former first lady, 75, admitted she is scared of losing her husband as the couple confronts a diagnosis that has already dramatically changed their lives.

“Of course I’m scared,” Jill said during an appearance on the Jaime Kern Lima Show on Thursday, Aug. 20. “I mean, first of all, he’s almost 10 years older than I am. And with an illness … you kinda can’t let your head go there.”

For Jill, the challenge has been acknowledging what could happen without allowing fear to consume the time she and Joe still have together.

“It’s like, you can’t imagine life without them,” she said. “But you know they’re going to die, just like I know I’m going to die. So part of it, you accept.”

The deeply personal comments come more than a year after Biden’s office revealed that the former president had been diagnosed with an aggressive form of prostate cancer.

Despite the seriousness of the illness, Jill said her husband has remained characteristically “strong” and “resilient.”

She also revealed that Joe has never directly told her whether he is afraid.

“He’s always been the strength of our family,” Jill said.

Rather than allowing the cancer diagnosis to dominate their lives, the Bidens have apparently made a deliberate decision to focus on the moments they still have together.

“We can’t look at life that way,” Jill explained. “We look at it like, ‘Joe has cancer. He’s gonna die, so let’s make the most of every moment.’”

That has meant spending more time together and actively searching for things that bring them happiness during one of the most difficult periods of their decades-long marriage.

“Let’s be together … let’s find things that bring us joy. Let’s make this a positive journey,” Jill said. “I mean, not a negative journey, because how can you live with that every day? You can’t. It’s too heavy.”

Biden’s cancer diagnosis was announced in May 2025 after doctors discovered an aggressive form of prostate cancer with a Gleason score of 9, also classified as Grade Group 5.

Even more troubling, the cancer had already spread to his bones by the time the diagnosis became public.

His office said the cancer was hormone-sensitive, however, meaning doctors had treatment options available to help manage the disease.

Jill has previously spoken openly about just how much the cancer’s spread changed the outlook for her husband.

During a June appearance on Today, she said Joe was “doing okay,” but acknowledged that metastatic cancer was far more serious than a prostate cancer diagnosis alone.

“If he had just been diagnosed with prostate cancer, that’s one thing, because that can be cured,” Jill said. “But the fact that it metastasized to his bones, that makes it a whole different story.”

She also made clear that the Bidens understand this could now be a lifelong battle.

“I think Joe will live with cancer ’til the rest of his life,” Jill said.

After nearly five decades of marriage — and years spent together through political triumphs, devastating family losses and the pressures of the White House — Jill’s latest remarks offered a rare glimpse into what the former first couple is facing behind closed doors.

For now, she says they are trying not to let fear dictate whatever time comes next.
